    Position Status: Permanent Full-Time

    Shifts: 12-hour Days/Nights Rotation

    Reports to: Care Manager, or designate


    PURPOSE OF ROLE AND STRATEGIC ALIGNMENT

    As a member of a multi-disciplinary team, the Registered Nurse provides leadership and directs nursing care of residents of the Chatham-Kent Hospice (CKH) according to established policies and procedures including: assessing and anticipating the total physical, emotional, spiritual and psychosocial needs of the residents; planning, documenting and implementing care plans and making recommendations for changes when necessary. Registered Nurses are expected to provide leadership and be a positive role model for staff, volunteers, guests, family members, health care professionals and the community.

    KEY RESPONSIBILITIES

    Respects and promotes the mission, vision and values of the Chatham-Kent Hospice


    Resident and Family Care

    1. Assesses, plans, implements and evaluates nursing care of residents
    2. Diagnoses actual or potential problems, plans and performs interventions and evaluates outcomes
    3. Collects information from a variety of sources using skills of observation, communication and physical assessment to enable the provision of appropriate nursing care
    4. Communicates and consults with members of the Hospice care team regarding resident care
    5. Participates in inter-professional conferences
    6. Acts as an advocate to protect and promote the resident and family rights to autonomy, respect, privacy, dignity and access to information
    7. Develops a plan of care in collaboration with the physician, the team, the resident and family
    8. Collaborates with hospice medical team and community primary care providers
    9. Documents timely and accurate reports of relevant observations including resident and family teaching and evaluation of nursing care
    10. Briefs staff and the leadership team and coordinates the admission of residents
    11. Assumes other related duties as assigned by the Care Manager


    Quality Assurance

    1. Attends staff meetings and educational sessions. If unable to attend, provides advance notice to Care Manager and reviews minutes of meeting
    2. Participates in quality improvement activities.
    3. Identifies self-learning needs and attends educational programs to maintain and enhance clinical competency
    4. Participates in the orientation and ongoing education of Hospice staff, students and volunteers by providing information and acting as a preceptor as assigned
    5. Participates in the evaluation of the resident care program, techniques, standards and procedures by providing feedback about effectiveness and resident care outcomes
    6. Participates in policy and procedure development, ensuring the Hospice complies with relevant Hospice Palliative Care Ontario (HPCO) and Canadian Hospice Palliative Care Association (CHPCA) standards, procedures, policies and guidelines


    Risk Management and Safety

    1. Maintains a safe environment for residents and families, staff and volunteers by adhering to established safety and emergency practices
    2. Adheres to safe medication practices and narcotics count controls in accordance with CKH policy and professional practice standards
    3. Complies with all CKH Clinical, Infection Control and Occupational Health and Safety policies and processes to eliminate or minimize unsafe acts or conditions, including identification and reporting of near misses and actual adverse occurrences
    4. Uses or wears any equipment, protective devices, or clothing required by CKH
    5. Reports to the Care Manager any known missing or defective equipment or protective device that may be dangerous to himself, herself, or another worker
    6. Commits to not using or operating any equipment or working in any way that may endanger any worker
    7. Ensures familiarity with emergency response plans and complies with the procedures outlined for each type of emergency at all times
    8. Reports any contravention of the Occupational Health & Safety Act or the regulations or the existence of any hazard to the Care Manager


    Leadership

    1. Provides shift leadership, serving as a positive role model for staff, volunteers, family members, visitors, health care professionals and the community.


    Privacy and Confidentiality

    1. Follows privacy and confidentiality policies under the Personal Health Information Protection Act, (PHIPA)


    Public Relations

    1. Raises community awareness of hospice palliative care and of the services provided by the Chatham-Kent Hospice


    Fundraising

    1. Acts as an ambassador to our community to help raise awareness for residential hospice care and our need for financial support
    2. Able to appropriately direct questions regarding support and/or donations and participates in CKH fundraising initiatives


    QUALIFICATIONS

    1. Member in good standing with the College of Nurses of Ontario (CNO) with a current Certificate of Registration as a Registered Nurse; BScN preferred
    2. A strong dedication to the vision, mission and values of CK Hospice
    3. Knowledgeable about and committed to the hospice philosophy of care
    4. Able to direct, delegate and work with staff RPNs, PSWs, students and volunteers
    5. Minimum three years recent palliative care experience with demonstrated expertise in pain and symptom management, preferred
    6. Fundamentals of Hospice Palliative Care completed within 6 months of hire and CAPCE Certificate initiated or completed within 1 year of hire, based on availability. LEAP considered an asset
    7. Certified in Hospice Palliative Care Nursing (Canada) (CHPCN) or willingness to pursue CHPCN(C) certification with the Canadian Nurses Association within 5 years of hiring, based on availability
    8. Excellent communication and interpersonal skills
    9. Proficiency in both official languages an asset
    10. Ability to work with resident/family living with and dying from advanced illness
    11. Commitment to ongoing learning and quality improvement
    12. Strong organizational and prioritization skills
    13. Demonstrated leadership skills with the ability to both give and take direction
    14. Ability to work collaboratively as part of a team and speak effectively in small group settings
    15. A strong and clear understanding of ethics, especially as related to hospice palliative care
    16. Highly developed problem solving and conflict management skills
    17. Current BCLS and commitment to renewal
    18. Physical ability to perform the duties of the position
    19. Ability to work rotating shifts
    20. Computer literate and able to work with Microsoft Suite
    21. Food Handling Course


    POLICE CHECK REQUIREMENTS:

    The nature of this position will require the successful candidate to provide a Vulnerable Sector Police Background Check prior to commencement of employment; please note that it may take several weeks to obtain a Police Information Search.


    HEALTH SCREENING REQUIREMENTS:

    Each member of staff must have a:

    1. 2 step TB test
    2. Tetanus


    COMPENSATION PACKAGE

    Based on qualifications and years of experience as outlined within the job description:


    Salary Range: $37.12 - $51.99 per hour based on years of experience

    Vacation: 2 weeks as outlined in Article 17.02 b of the CLAC Local 303 Collective Agreement

    Pension: Healthcare of Ontario Pension Plan (HOOP)

    Benefits: Comprehensive benefit plan includes prescription drug coverage, vision care, health care, dental care, and life insurance/accidental death and dismemberment
